About this Job
As part of your role, you will: Contact “Immediate Outreach” members
Assist members with mobile care management devices
Communicate with members to identify risks and educate on recommended ranges of their devices
Educate members about concerning symptoms and options for outreach – including when to refer to the ER or ER alternatives
Maximize member’s access to available resources
Assist health coaches, customer service and tech support team as needed
Collaborate and communicate with entire care team to best support members

Required
Current & Active Registered Nurse (RN) licensure in the following “states:” (District of Columbia, Illinois, Ohio, Michigan, New Hampshire, Delaware, Nevada, New York, Pennsylvania)
Minimum 1-3 year(s) of experience in critical care setting
Knowledge of chronic disease management and progression (Diabetes, Hypertension, Congestive Heart Failure, Asthma, COPD, Weight Management, Mental/Behavioral Health)
Proficiency with technology/computers in web-based applications
Ability to work closely with member and care team
Effective communication skills
Compassion and empathy
Exceptional interpersonal, organization and communication skills required

Preferred
Emergency Room experience
Work with diverse populations
Medicaid and/or Medicare knowledge
Telephonic patient outreach
Motivational Interviewing
Care Coordination or Case Management
